Are you prescribing e-liquids for e-cigarettes?

What doctors should consider before prescribing e-liquids for e-cigarettes an article in the Medical Journal of Australia asks - An Australian standard for prescribing e-cigarette nicotine has arrived — are you prepared with the facts?

Various nicotine replacement therapies have been approved by the Therapeutic Goods Administration as safe and effective smoking cessation tools. Many smokers are now using e-cigarettes to reduce their tobacco consumption even though there is no clear unbiased data on whether and how effectively they help. There is however a growing body of evidence that shows they may cause harm. Evidence has emerged showing that using both tobacco cigarettes and e-cigarettes may be worse then doing either alone, particularly for heart health.

Read the whole very informative article that details the effects of flavourants on nictine absorbtion that could in fact increase nicotine addiction. They stress that it is critical for physicians to be aware of these effects and to recommend prescribing with structured behavioural support to promote tobacco cessation and ideally complete nicotine abstinence.

Original research - Calling an ambulance for non-emergency medical situations.

 An original research article recently published in Emergency Medicine Australia, Calling and ambulance for non-emergency medical situations : Results of a cross-sectional online survey from an Australian nationally representative sample, by Brennen Mills, Michaella Hill, Alecka Miles, Erin Smith, Eben Afrifa-Yamoah, David Reid Shane Rogers and Moira Sim investigates the general public's perception of medical scenarios that warrant a call to an emergency ambulance.

17 hypothetical scenarios were presented with participants being asked to decide which scenarios they would call an ambulance for. There were several non-emergency scenarios that 40% of the population incorrectly identified as warranting and emergency ambulance.

The authors find that while risk aversion is common among the populace a trait that should not necessarily change, an appropriate balance between risk aversion and overreliance on an already significantly stretched emergency health system needed to be found. The paper identifies contributors to engagement with emergency health services including demographics of the population sample, GP access and wider reaching public health problems, and suggests interventions that could help including education and guidance about the clinical urgency of symptoms and the appropriate pathways to take in seeking help.

Year 4 of COVID-19 in Australia

 As Australia fights the most recent COVID-19 wave, Aussies will be hoping 2023 is the year the pandemic "ends", the year when figuring out exposure risks, decisions on mask use, missing out on social events due to that little tickle in the throat, can all be put behind us.

Medical experts say it's vital the community understands that isn't the case — but there are some things that could make this year a bit easier than the past three.

In 2022, the highly infectious Omicron variant made its arrival felt in Australia's pandemic, seeing cases rise steeply.

Australia has seen thousands of COVID-19 deaths in 2022, making the the tolls from 2020 and 2021 seem insignificant.

The surge in cases, peaking in July 2022, has put a huge amount of pressure on health systems, COVID-19 cases requiring hospitalisation increased and in turn the nations healthcare workers are forced to isolate as they too become infected. Cases requiring hospitalisation were down at the beginning of November but the end of the year saw cases on the rise again due to new variants emerging in the past 3 months.

The emphasis needs to be on the basics to reduce the spread of the virus. A key difference in 2022 was the shift from imposed restrictions to personal responsibility. Australia has gone from an extremely high perception on risk to a point where some people are complacent and to not appreciate that risk still exists in the community.

Strategies that everyone can put in place : Meeting up outdoors if possible, ventilating indoor spaces well, using a mask in high-risk settings such as busy shopping malls or supermarkets, keeping up-to-date with vaccines and staying home if you are unwell.

COVID-19 is evolving but so are our vaccines. 

The trend in evolution at the moment is that the virus is becoming more transmissible but not more severe. The danger is when a variant emerges that is more severe resulting in higher hospitalisation and mortality.

Injury in Australia - Falls Statistics and Prevention

 A recent report "Injury in Australia" by the Australian Institute of Health and Welfare highlights that falls were overwhelmingly the the single contributor to hospitalisations in the year 2020-21. The majority of falls occurred on a single level surface, not stairs as many may think. In 2019 to 2020 over 5000 people sadly died as a result of a fall.

People aged 65 and over were more likely to be hospitalised or die because of a fall compared with other age groups. Females are more likely than males to be hospitalised or die as a result of a fall. Read on in the linked website for more information.

Falls prevention should be at the forefront of the minds of  anyone working with people in the at risk age group. The Australian Commission on Safety and Quality in Healthcare have produced a guideline, Preventing Falls and Harm from Falls in older People : Best Practice guidelines for Australian Hospitals.

MNCLHD Staff - Access to articles and E-Resources

 MNCLHD STAFF SOURCES OF INFORMATION

Using the Library catalogue is a great way to check if what you want is available through the MNCLHD Library Service. You can search the site without registering but to use the resources you must be registered as library member. Why? - Because the catalogue is hosted on an internet site you must be verified as a legitimate user. You can register through the catalogue itself or through the Library's Intranet page . It will take a day or two to be confirmed so if you're going to use this resource register ASAP. Thousands of journals and e-books are on hand. Using the advanced search area or 'Lists" can help narrow down searches to the resources you need.

NSW Health Clinical Information Access Portal, CIAP is another great resource to keep in mind. Use the "Journals" or "Books and Dictionaries" area to search for journal or book titles. You can also search by subject. If you do not have a specific article of book in mind you can use database area to search for resources in your area of research or interest. Full-text items may be available directly through CIAP or you can 'Request the Article". These  article request come to the library if you're unsure where they go to. Your requests will be dealt with quickly, normally within 48 hours.

Did you know that sometimes articles can be found full-text through Google Scholar? Google Scholar has linked CIAP holdings. That means that if you search for the article by title, the link for CIAP will come up with a direct link to the full-text.
