** Reminder ** MNCLHD Libraries Needs Assessment Survey

We are carrying out an evaluation of the library’s services, to see if we can improve facilities and services and keep them up to date with staff needs. Thank you for taking the time to fill in this questionnaire; it should only take five – ten minutes of your time. Your answers will be treated with complete confidentiality and will be entirely anonymous. Hardcopies are available in both CHHC and PMBH library.
